The chemical name of Almirol is pentosan polysulfate sodium, which is a semi-synthetic heparin-like macromolecular carbohydrate derivative that is chemically and structurally similar to glycosaminoglycans. This medication is mainly used to relieve bladder pain or discomfort caused by interstitial cystitis.
Precautions for Amirol
1. Hematological effects
It is a weak anticoagulant, and bleeding complications such as rectal bleeding and ecchymoses, nosebleeds, and gum bleeding have been reported. Patients at increased risk of bleeding (including those undergoing invasive procedures, those with symptoms or signs of coagulopathy, or those taking concomitant medications that affect hemostasis) need to be evaluated.
There have been reports that when other dosage forms of Almirol are administered subcutaneously, intramuscularly or sublingually, a delayed immunoallergic reaction similar to heparin-induced thrombocytopenia may occur, manifesting as thrombosis and bleeding symptoms.
Use with caution in patients with a history of heparin-induced thrombocytopenia. Patients with thrombocytopenia should be carefully evaluated before treatment.
thrombocytopenia and prolonged prothrombin time (PT) and partial thromboplastin time (PTT) have been reported in patients with elevated liver function test results. In healthy men, no such effects were observed when ≤1.2 g of pentosan polysulfate sodium was used daily (above the recommended dose of 100 mg three times daily) for 8 days.

2. Concomitant diseases
Patients suffering from aneurysms, hemophilia, gastrointestinal ulcers, polyps or diverticula need to be carefully evaluated before treatment.
3. Liver effects
About 1.2% of patients experience mild and usually transient increases in serum aminotransferase, alkaline phosphatase, γ-glutamyl transpeptidase and lactate dehydrogenase (LDH) concentrations (<2.5 times the upper limit of normal values).
The above abnormalities usually occur within 3-12 months after the start of treatment, and are usually not accompanied by jaundice or other clinical symptoms and signs. These increases may remain unchanged with continued medication or, rarely, may progress.
4. Hair loss
There have been reports of hair loss with Amero, mainly alopecia areata (limited to a single area of the scalp), which may occur within the first 4 weeks of treatment.
Amiron medication for specific groups of people
1. Pregnancy
Pregnancy classification Category B.
2. Lactation period
It is not clear whether pentosan polysulfate is excreted in breast milk, and lactating women should use it with caution.
3. Pediatric medication
The safety and effectiveness of using Amirol in children under 16 years of age has not yet been established.
4. Hepatic insufficiency
This group of people should use Almiro with caution. It has not been evaluated in patients with hepatic insufficiency. However, the hepatic metabolism of Almiro is affected by liver function, and its original pharmacokinetics may also change to some extent.
Common adverse reactions of Amirol
Mainly include rectal bleeding, hair loss, diarrhea, nausea, headache, blood in the stool, rash, indigestion, abdominal pain, abnormal liver function, dizziness, and bruises.
Contraindications of Almirol
Contraindicated in persons with known allergy to pentosan polysulfate, structurally related compounds, or any component of the preparation.
