Baxdrostat: Efficacy and Safety of a Novel Aldosterone Synthase Inhibitor
Baxdrostat(brand name Baxfendy)is a novel aldosterone synthase inhibitor primarily developed for patients with uncontrolled or resistant hypertension.Unlike direct receptor blockers,it suppresses aldosterone production at the synthesis stage, reducing sodium retention and lowering blood pressure.
By targeting the aldosterone synthesis pathway, baxdrostat effectively decreases endogenous aldosterone levels, offering a new therapeutic approach distinct from traditional aldosterone receptor antagonists.
The Phase III BaxHTN trial, published in NEJM in 2025,enrolled 794 patients with uncontrolled or resistant hypertension. After 12 weeks, the baxdrostat 2 mg group showed a 15.7 mmHg reduction in seated systolic blood pressure from baseline(compared to 5.8 mmHg in the placebo group),resulting in a placebo-corrected reduction of 9.8 mmHg.The 1 mg group achieved a corrected reduction of 8.7 mmHg,both statistically significant.
For resistant hypertension, the Phase III Bax24 trial further confirmed its efficacy. After 12 weeks of treatment, the 2 mg group achieved an additional 14.0 mmHg reduction in 24-hour ambulatory mean systolic blood pressure compared to placebo.Significant reductions were observed in both nighttime and daytime blood pressure, indicating round-the-clock efficacy.
While baxdrostat demonstrates potent antihypertensive effects, safety monitoring is crucial. In the BaxHTN trial,3.0%of patients in the 2 mg group experienced serum potassium levels exceeding 6.0 mmol/L(vs.0.4%in placebo),highlighting the risk of hyperkalemia and hyponatremia. Therefore, balancing blood pressure control with electrolyte safety is essential in clinical practice.
