埃索美拉唑的注意事项

Esomeprazole (Nexium) is a proton pump inhibitor that was launched in Sweden in 2000 and approved by the FDA in the United States in 2001. What are the precautions for esomeprazole?

1. When any alarm symptoms (such as significant unintentional weight loss, repeated vomiting, dysphagia, vomiting blood or melena) occur, and gastric ulcer is suspected or already has gastric ulcer, malignant tumors should be excluded, because treatment with esomeprazole magnesium enteric-coated tablets can reduce symptoms and delay diagnosis.

2. Patients who have been treated with this drug for a long time (especially those who have used it for more than 1 year) should be monitored regularly.

3. Patients receiving on-demand treatment should be informed to contact their physician if the characteristics of their symptoms change. During on-demand treatment, possible drug interactions due to fluctuations in esomeprazole plasma concentrations should be considered.

4. When esomeprazole is used for Helicobacter pylori eradication treatment, possible drug interactions of all components of triple therapy should be considered. Clarithromycin is a potent inhibitor of CYP 3A4, so the contraindications and interactions of clarithromycin should be considered when patients on triple therapy are concurrently taking other drugs that are also metabolized by CYP 3A4, such as cisapride.

5. Patients with rare hereditary diseases such as fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase deficiency should not take this product.

6. Renal impairment: Patients with renal impairment do not need to adjust the dose. For patients with severe renal insufficiency, caution should be used during treatment due to limited experience with this drug.

7. Hepatic impairment: Patients with mild to moderate hepatic impairment do not need to adjust the dosage. For patients with severe hepatic impairment, the dose of esomeprazole magnesium enteric-coated tablets should not exceed 20 mg.

8. Effects of esomeprazole (Nexium) on the ability to drive and use machines. No effects in this area have been observed.
