It is a selective high-affinity antagonist of the human substance P neurokinin 1 (NK1) receptor. Substance P and its receptors are new targets for clinical treatment of nausea and vomiting in patients with malignant tumors after chemotherapy. The so-called substance P is actually a polypeptide chain containing 11 amino acids, which together with NK (neurokinin A) and NK-B (neurokinin B) belong to the tachykinin family. The receptors of this family include three subtypes: aprepitant, NK-2, and NK-3. Substance P and its immune reactants are widely present in chromaffin cells, nucleus of the solitary tract, vagus nerve, area postrema and other key sites that produce vomiting after chemotherapy. It mainly binds to aprepitant receptors located in the intestine, central nervous system and peripheral nerves, and then exhibits a variety of biological effects.

Substance P has the effect of stimulating aprepitant to induce vomiting, and aprepitant receptor antagonists mainly bind to aprepitant receptors, thereby blocking the action point of substance P. Aprepitant receptor antagonists can bind to aprepitant receptors in the brain through the blood-brain barrier with high selectivity and affinity, but with low affinity for the other two pressure types.

By selectively antagonizing aprepitant receptors, it blocks nausea and vomiting caused by cytotoxic substances. Currently, some clinical researchers say that aprepitant receptor antagonists have a broader spectrum of activity against various emetics than 5-HT3 receptor antagonists, and have found that they have more prominent clinical effects on delayed vomiting.

How many times a day should you take aprepitant for chemotherapy? When to eat?

Aprepitant needs to be taken orally 125 mg 1 hour before chemotherapy and 80 mg in the morning on days 2 and 3, once a day.
