Indications of aprepitant:

Aprepitant, administered in combination with other antiemetics, is indicated for the prevention of acute and delayed nausea and vomiting during initial and repeated treatment with highly emetogenic antineoplastic chemotherapy.

The effects of aprepitant:

Aprepitant is a selective high-affinity antagonist of the human substance P neurokinin 1 (NK1) receptor. Other existing drugs for the treatment of chemotherapy-induced nausea and vomiting (CINV) and postoperative nausea and vomiting (PONV) have low or no affinity for the targets 5-hydroxytryptamine receptor 3 (5-HT3), dopamine receptors and glucocorticoid receptors. Aprepitant inhibits cisplatin-induced acute and delayed phase vomiting and enhances the antiemetic activity of the 5-HT3 receptor antagonist ondansetron and the glucocorticoid dexamethasone against cisplatin-induced vomiting.

Efficacy of aprepitant:

In a multicenter, randomized, double-blind controlled clinical trial, patients treated with high-dose (≥70 mg·m) cisplatin experienced highly emetogenic reactions after receiving chemotherapy. The control group received an intravenous injection of 20 mg ondansetron on day 1 and oral administration of 20 mg dexamethasone at the same time; 8 mg·d dexamethasone was taken twice on days 2 to 4. The experimental group received 12 mg dexamethasone and 125 mg aprepitant orally on day 1, 8 mg dexamethasone on days 2-4, and 80 mg·d aprepitant on days 2 and 3. It shows that the effect of the test group is obviously better than that of the control group; taking aprepitant can significantly delay the time of nausea, and it also has a good effect on delayed vomiting.

Clinical data show that aprepitant can significantly and effectively prevent acute nausea and vomiting caused by chemotherapy, and its outstanding advantage is that it can also prevent vomiting and delayed vomiting during chemotherapy.
