Shudan Ondansetron Hydrochloride Tablets are a potent and highly selective 5-HT3 receptor antagonist with strong antiemetic effects. Chemotherapy drugs and radiotherapy can cause the release of 5-HT in the small intestine and activate the afferent branch of the vagus nerve through the 5-HT3 receptor, triggering the vomiting reflex. This product can block this reflex and trigger the afferent branch of the vagus nerve. It can also cause the release of 5-HT in the Postrema area at the bottom of the fourth ventricle, thereby strengthening the effect of this product on the central mechanism. For nausea and vomiting caused by chemotherapy and radiotherapy, this product exerts an antiemetic effect by antagonizing the 5-HT receptors of local neurons located in the peripheral and central nervous system. The mechanism of post-operative nausea and vomiting is unknown, but it may be induced by a common pathway similar to that of cytotoxic drugs that induce nausea and vomiting. This product can also inhibit opioid-induced nausea, and its mechanism of action is still unclear. Due to its highly selective effect, this product does not have the side effects of other antiemetics, such as excessive sedation for extrapyramidal reactions.
The clinical trial was conducted on 60 postoperative patients with esophageal cancer who received cisplatin combined with chemotherapy, using a randomized controlled method to compare the efficacy and toxicity of Shudan alone, metoclopramide alone, and both combined with dexamethasone. The results showed that the antiemetic rates of the Shudan group, the metoclopramide group and the combined group were 82.5%, 67.5% and 95.0% respectively, and the complete remission rates were 72.5%, 60% and 87.5% respectively. The difference between the antiemetic metoclopramide group and the combined group was statistically significant. The toxic and side effects were milder in the combination group. Preliminary research results show that the combined use of chemotherapy antiemetics, metoclopramide, and dexamethasone is the most effective solution for preventing nausea and vomiting caused by adjuvant chemotherapy after surgery for esophageal cancer.
