What are the side effects of Dinutuximab?
Dinutuximab (Dinutuximab) is a monoclonal antibody drug targeting the GD2 antigen and is widely used to treat high-risk neuroblastoma (neuroblastoma). Although the drug provides a clinically effective treatment option for many patients, as with all drugs, use of datuximab may be associated with a range of side effects. Understanding these side effects is very important for doctors, patients and family members to help them take adequate preventive and response measures during the treatment process.
1. Neuropathic pain
Neuropathic pain is one of the most common side effects of datuximab, especially early in treatment. Because the drug works by targeting the GD2 antigen in the nervous system, neuropathic pain that may occur during treatment often manifests as a tingling or burning sensation in the limbs, trunk, and face. The intensity of pain varies from patient to patient, and some patients may need analgesics to relieve the discomfort. To reduce the occurrence of neuropathic pain, doctors often prescribe pain medications, such as opioids or nonsteroidal anti-inflammatory drugs (NSAIDs), before treatment and closely monitor patients' symptoms during treatment.
2. Fever and chills
Fever and chills are another common side effect during treatment. Approximately 80% of patients receiving datuximab develop fever, usually within the first few days of treatment. A febrile reaction is usually caused by an immune response triggered by medications and is a normal immune response but can also cause discomfort. Fever may be accompanied by symptoms such as chills, headache, chills, and chills, which usually resolve on their own within a few hours. However, if the fever is more severe, fever-reducing drugs (such as acetaminophen) or anti-inflammatory drugs may be needed to control the temperature and ensure that the patient's body temperature does not get too high.

3. Allergic reaction
Anaphylaxis is a possible side effect of any monoclonal antibody drug, and datuximab is no exception. Some patients may develop skin irritation, difficulty breathing, rash, or severe allergic reaction (such as anaphylactic shock) while using this drug. Anaphylaxis usually occurs when the drug is first administered, so doctors usually observe the patient closely at the beginning of treatment and have appropriate first aid measures ready, such as epinephrine and anti-allergy medications (such as antihistamines). In order to prevent the occurrence of allergic reactions, patients are usually given anti-allergic drugs before treatment, and the drug dosage is adjusted or treatment is suspended during treatment.
4. Hypotension and cardiovascular discomfort
Hypotension is one of the common side effects of datuximab, especially early in the drug infusion. As the drug triggers an immune response, it may lead to vasodilation and hemodynamic changes, resulting in hypotension. Severe hypotension may cause patients to feel dizzy, weak, or faint, affecting their normal life. In order to relieve hypotension, patients are usually given fluids during treatment, and vasopressor drugs are used when necessary to maintain blood pressure within the normal range.
In addition, some patients may experience abnormal heart rate or electrocardiogram changes during treatment. Therefore, patients need to be monitored with electrocardiogram during treatment. If heart problems are found, the treatment plan needs to be adjusted in time and necessary treatment must be carried out under the guidance of a doctor.
5. Abnormal liver function
During datuximab treatment, some patients may develop liver function abnormalities, especially in the early stages of treatment. Blood tests may show elevated liver enzymes such as ALT and AST, which may be due to an immune response to the liver. Although these changes are usually temporary, they may cause liver damage in some patients. For this reason, liver function needs to be monitored regularly during treatment. Once abnormalities are found, the treatment plan needs to be adjusted according to the doctor's recommendations, which may include suspending treatment or using drugs to protect the liver.
6. Reduces blood cells
Some patients may experience a decrease in white blood cells, red blood cells, or platelets while being treated with datuximab, which is called cytopenia. Decreased blood cells may lead to a decrease in immune system function, increase the risk of infection, or lead to anemia and bleeding tendencies. Therefore, during treatment, doctors will regularly monitor the patient's blood routine in order to detect and deal with the problem of blood cell reduction in time. For some patients, medication may need to be suspended or adjusted, and supportive care may need to be given, such as blood transfusions or antibiotics to prevent infection.
7. Other side effects
In addition to the common side effects mentioned above, datuximab may also cause some rare side effects, including gastrointestinal reactions (such as nausea, vomiting, diarrhea), fatigue, headache, loss of appetite, etc. These side effects are usually mild and gradually resolve with treatment. In order to improve the patient's quality of life, doctors may give symptomatic treatment to relieve uncomfortable symptoms.
Although datuximab is an effective immunotherapy drug, it comes with a certain risk of side effects. During the treatment process, patients need to strictly monitor and deal with possible side effects in a timely manner under the guidance of a doctor. Through reasonable drug regulation, monitoring and supportive treatment, most side effects can be effectively controlled and alleviated, thereby ensuring the safety and effectiveness of treatment. Patients and their families should maintain close communication with their doctors to jointly deal with problems that may arise during treatment to ensure the smooth progress of treatment.
