2. Indications:
Teriflunomide (Teriflunomide)is indicated for the treatment of relapsing forms of multiple sclerosis (MS) in adults, including clinically isolated syndrome, relapsing-remitting disease, and active secondary progressive disease.
3. Usage and dosage:
1. Before treatment: Within 6 months before starting teriflunomide treatment, patients need to obtain transaminases, bilirubin levels, complete blood count(CBC), blood pressure, and use tuberculin skin test or Mycobacterium tuberculosis infection blood test to screen the patient for latent tuberculosis infection.
2. Recommended dose: The recommended dose of teriflunomide is 7 mg or 14 mg, taken orally once a day. Monitor alanine aminotransferase (ALT) levels at least monthly for six months after patients start taking teriflunomide.
4. Adverse reactions:
In clinical studies,the most common adverse reactions of teriflunomide are headache, increased alanine aminotransferase (ALT), diarrhea, hair loss and nausea. The most common adverse reactions related to discontinuation are increased ALT; this was discovered during the post-marketing use of teriflunomide. The occurrence of adverse reactions such as thrombocytopenia, pancreatitis, colitis, drug-induced liver injury (DILI), allergic reactions and angioedema, interstitial lung disease with eosinophilia, toxic epidermal necrolysis and Stevens-Johnson syndrome, psoriasis or exacerbation of psoriasis (including pustular psoriasis and nail psoriasis), and nail diseases.
5. Storage:
Teriflunomide is available in 7 mg and 14 mg tablets and is stored at 20-25°C (68-77°F).
6. Taboo:
Teriflunomide is contraindicated in patients with severe hepatic impairment, pregnant women and women of childbearing potential who are not using effective contraception, or who have a history of allergic reaction to teriflunomide, leflunomide, or any of the inactive ingredients in teriflunomide. Teriflunomide is prohibited from being used in combination with leflunomide.
7. Mechanism of action:
Teriflunomide is an immunomodulator with anti-inflammatory properties that inhibits dihydroorotate dehydrogenase (a mitochondrial enzyme involved in de novo pyrimidine synthesis). The exact mechanism by which teriflunomide exerts its therapeutic effect in multiple sclerosis is unknown but may involve a reduction in the number of activated lymphocytes in the central nervous system.

It may be limited to multiple sclerosis patients who are ineffective in conventional treatments.
