How to use Cobenfy (KarXT)

1. Dose escalation scheme (titrated administration)

(1). Starting dose: 50mg/20mg (xanomeline/trospium chloride), taken orally, twice a day for at least 2 days.

(2) First increase: increase to 100mg/20mg, orally, twice a day for at least 5 days.

(3) Target dose: According to the patient's tolerance and treatment response, it can be further increased to 125mg/30mg, orally, twice a day.

2. Key dosing timing

(1) Take on an empty stomach: To ensure drug absorption, it should be taken at least 1 hour before a meal or at least 2 hours after a meal.

(2) Swallow the whole capsule: Do not open, chew or crush the capsule. Take the whole capsule with warm water.

3. Dose adjustment for special groups

Elderly patients: The recommended starting dose is 50mg/20mg twice daily, the maximum dose should not exceed 100mg/20mg twice daily, and a slower dose escalation rate should be adopted.

Contraindications for Cobenfy (KarXT)

The following patients are absolutely prohibited from using Cobenfy, otherwise it may lead to serious or even life-threatening consequences:

1. Patients with urinary retention: Cobenfy may aggravate the condition and cause the inability to urinate.

2. Patients with moderate to severe liver dysfunction: refers to patients with Child-Pugh classification B or C. After use, the blood concentration of the drug will significantly increase, significantly increasing the risk of adverse reactions.

3. Patients with gastric retention: Drugs will slow down gastrointestinal motility and aggravate delayed gastric emptying.

4. Patients allergic to ingredients: patients with a history of allergy to Cobenfy, Xanomeline, Trospium Chloride or any of the excipients.

5. Patients with untreated narrow-angle glaucoma: The anticholinergic effect of the drug may induce acute glaucoma attacks.
