Antihypertensive drug instructions

Eplerenone is the first selective aldosterone receptor antagonist. It was approved for marketing in the United States in 2002. It is currently used to treat hypertension and left ventricular dysfunction after acute myocardial infarction. It can effectively lower blood pressure, improve cardiac function, and reduce myocardial damage. It has not yet been marketed in China.

Indications: It is used to treat patients with hypertension, and it can also be used for congestive heart failure after acute myocardial infarction.

Usage and dosage: When treating adult patients with hypertension, eplerenone can be used alone or in combination with other antihypertensive drugs. It is administered orally. The initial dose is 50 mg, once a day; the maintenance dose is 50 mg, 1-2 times a day. The maximum dose is 100 mg/day and patients should not overdose. Typically, significant blood pressure lowering effects occur within four weeks of taking eplerenone. Patients whose blood pressure responds inadequately to the initial dose may be increased to 50 mg twice daily.

The initial dose of eplerenone for the treatment of congestive heart failure is 25 mg orally once daily; the dose can be increased to the target dose within four weeks as tolerated by the patient. The target dose is 50 mg orally once daily.

Adverse Reactions: Common adverse reactions in >10% are endocrine and metabolic: hyperkalemia, hypertriglyceridemia (1%-15%; dose related).

1%-10% of adverse reactions are central nervous system: fatigue, dizziness. Endocrinology and Metabolism: Hyponatremia (dose-related), proteinuria, hypercholesterolemia, gynecomastia. Gastrointestinal tract: diarrhea, abdominal pain. Respiratory system: flu-like symptoms, cough. Genitourinary system: abnormal vaginal bleeding, breast pain (males: ≤1%). Kidney: Increased serum creatinine.
