It is a mineralocorticoid receptor antagonist that competes with aldosterone. Aldosterone receptors in late distal renal tubules and renal collecting ducts can induce sodium reabsorption and potassium excretion from the distal renal tubules. Inhibiting this receptor causes sodium ions to be excreted in the urine while maintaining potassium levels in the body.

Compared to spironolactone, eplerenone has a higher affinity for aldosterone receptors and is associated with less anti-adrenergic reactions (gynaecomastia and alopecia). However, the molecular structures of the two drugs are very similar.

Eplerenone was approved for marketing in the United States in 2002 for the treatment of hypertension, and was subsequently used to improve the survival rate of heart failure patients with myocardial infarction. So, has eplerenone been marketed in China?

Eplerenone has not been officially launched in China, but it was approved for clinical use by the State Food and Drug Administration in 2002. Now it is generally favored by domestic doctors. Its effect is stronger than spironolactone and has almost no side effects. And it can be used for congestive heart failure after acute myocardial infarction: eplerenone can improve the quality of life of patients with left ventricular dysfunction (ejection fraction ≤ 40%). Clinical trials have proven that eplerenone can also be used for congestive heart failure after acute myocardial infarction. Of course, eplerenone can also be used for the control and treatment of common hypertension.

Precautions for eplerenone:

Eplerenone should not be combined with potassium supplements, potassium-containing salts, or contraindicated drugs (potassium-sparing diuretics, strong inhibitors of CYP450 liver enzymes).

Non-steroidal anti-inflammatory drugs and lithium can affect the blood concentration of eplerenone and affect the efficacy.

People with high serum potassium, type 2 diabetes patients with microalbuminuria, high serum creatinine levels, and muscle intoxication clearance <50 mL/min should not take it. Pregnant women, children, and those with hepatic and renal insufficiency should use with caution.
