Research on the use of Inspra in the treatment of early-stage cardiomyopathy in DMD: This study mainly wants to explore the efficacy of using Inspra in DMD patients who have already developed myocardial damage under the premise of applying basic treatment.

Boys with DMD aged 7 years and older from three clinical centers in the United States participated in the trial. All DMD patients who were diagnosed with myocardial damage through enhanced cardiac MRI and ejection fraction were also given oral administration of 25 mg Inspra or placebo on the basis of treatment with ACEI or ARB drugs, once every other day in the first month, and once daily after one month. The primary outcome of this study was the change in left ventricular circumferential strain (Ecc), an index of myocardial contractile function, measured at 12 months. Throughout the entire process, its safety was determined by continuously measuring serum potassium and cystatin C (another indicator of renal function in addition to creatinine).

Results: A total of 188 DMD patients were screened, and 42 DMD patients were included in this clinical trial. 42 people were randomly assigned, of which 20 patients received Inspra treatment and 22 patients received placebo treatment. Finally, 20 patients in the Inspra group and 20 patients in the placebo group completed the 0, 6, and 12-month evaluations. At 12 months, left ventricular circumferential strain decreased less in the Inspra group than in the placebo group. Cystatin C and serum potassium concentrations were within normal ranges in both groups.

Discussion: In patients with DMD, treatment on the basis of existing ACEI or ARB therapy can reduce the progression of decline in left ventricular systolic function. Early application of effective drugs can reduce the risk of death from cardiac causes, but the impact of combined use of cardioprotective drugs on the survival rate of DMD patients needs further study.
