It can be used alone or in combination with other antihypertensive drugs for the treatment of hypertension. The clinical trial explores the effects of eplerenone and losartan on arterial stiffness and central aortic pressure (CAP) in patients with dual-stage mild to moderate essential hypertension with elevated systolic and diastolic blood pressure. Eplerenone is not currently on the market in my country, so eplerenone is not included in medical insurance.

  The trial included 84 patients with mild to moderate essential hypertension [diastolic blood pressure 90-110mm Hg (1mm Hg=0.133kPa) and systolic blood pressure 140-180mm Hg] were divided into eplerenone group and losartan group, with 42 patients in each group. After taking placebo for 2 weeks, they entered the 12-week trial period. They were given eplerenone 50 mg or losartan 50 mg orally once/d, and followed up with a follow-up visit once/2 weeks. After 4 weeks, if the diastolic blood pressure ≥90 mm Hg, they would be changed to eplerenone 100 mg orally divided into 2 times or losartan 100 mg orally once/d.

Results: 37 patients with hypertension in the eplerenone group completed the trial and 5 were lost to follow-up; 40 patients in the losartan group completed the trial and 2 were lost to follow-up; compared with those before treatment, the sitting systolic and diastolic blood pressures, ambulatory blood pressure parameters, baPWV and C of patients in the eplerenone group and losartan group AP was significantly reduced; at the end of 12 weeks of treatment, there was no significant difference in the decrease in sitting systolic blood pressure and diastolic blood pressure between the two groups [(18.3±8.6) vs. (21.4±9.4), (14.0±5.3) vs. (15.1±6.6)mm Hg, both P<0.05]; there was no statistically significant difference in the 24-hour average systolic blood pressure and average diastolic blood pressure, daytime average systolic blood pressure and average diastolic blood pressure, nighttime average systolic blood pressure and average diastolic blood pressure, baPWV, CAP and total adverse event incidence in the eplerenone group compared with the losartan group.

Usage and Dosage: Common dosage for adults with congestive heart failure, initial dose: 25 mg, orally, once a day; and within 4 weeks, gradually increase to the target dose as tolerated by the patient, target dose: 50 mg, orally, once a day.
