It is a new type of selective aldosterone receptor antagonist that was approved for clinical use by the US National Food and Drug Administration in 2002. Eplerenone is mainly used for the treatment of congestive heart failure and hypertension after acute myocardial infarction. The FDA (U.S. Food and Drug Administration) has approved eplerenone for use in stable patients with left ventricular systolic dysfunction (ejection fraction <40%) and congestive heart failure after acute myocardial infarction (MI) to improve survival rates. Eplerenone is the first aldosterone receptor blocker approved for this indication. The FDA's approval was based on the results of the EPHESUS (Eplerenone Efficacy and Survival Study in Heart Failure After Acute Myocardial Infarction) trial. In patients with heart failure after myocardial infarction, eplerenone plus standard care reduced mortality by 15% compared with placebo and standard care (ACE inhibitors and beta-blockers).
How much eplerenone should be used?
1. Congestive heart failure after acute myocardial infarction: The recommended dose of eplerenone is 50 mg/time, once/day. The initial dose should be 25 mg/time, once/day, and within 4 weeks, the dose should be gradually increased to 50 mg/time, once/day if the patient tolerates it.
2. Anti-hypertensive: Eplerenone can be used alone or in combination with other anti-hypertensive drugs. The recommended initial dose of eplerenone alone is 50 mg/time, once/day. Obvious pressure-reducing effect will appear within four weeks of taking the medicine. If the pressure effect is not obvious, it can be increased to 50 mg/time, 2 times/day. Higher dosages are not recommended, otherwise there is an increased risk of adverse reactions such as hyperkalemia.
The side effects include: The main side effects are: cough, diarrhea, abdominal pain, angina, myocardial infarction, proteinuria, headache, dizziness, fatigue, flu-like symptoms, etc. Other side effects include: gynecomastia, abnormal vaginal bleeding; increases (serum potassium, triglycerides, cholesterol, alanine aminotransferase, γ-glutamyl transferase, creatinine, uric acid levels), decreased serum sodium levels, etc.
