Opicapone Drug Interactions and Medication Guide to Avoid Conflicts
Opicapone (Opicapone) is a selective, long-acting catechol-O-methyltransferase (COMT) inhibitor. It is mainly used in patients with Parkinson's disease in combination with levodopa to prolong its efficacy. In clinical use, reasonable avoidance of drug interactions is crucial to ensure therapeutic efficacy and reduce the risk of adverse reactions. The following is four-point guidance on drug interactions and avoiding conflicting medication with Opicapone during medication:
First, opicapone should not be used concurrently with other COMT inhibitors (such as entacapone or tolcapone). The combined use of multiple COMT inhibitors can potentiate the effect of levodopa and increase levodopa-related adverse reactions, such as dyskinesia, hypotension, or hallucinations, so such combinations should be avoided. If you need to replace the COMT inhibitor, you should gradually replace it under the guidance of a doctor. It is not recommended to adjust the plan by yourself.

Secondly, opicapone can enhance the efficacy of levodopa, so you need to be alert to the increase in levodopa-related side effects, such as motor fluctuations, dyskinesias, or psychiatric symptoms during combined treatment. In this case, appropriate adjustment of the levodopa dose may be necessary. At the same time, avoid using it with other drugs that affect dopamine metabolism (such as non-selective monoamine oxidase inhibitorsMAOIs) to avoid excessively high dopamine levels and inducing severe hypertensive crisis or mental disorder.
Third, caution should be taken when combined with drugs that affect the CYP450 enzyme system. Although the metabolic pathway of opicapone is relatively clear, there are still potential enzyme-inhibiting or inducing effector drugs (such as ketoconazole, carbamazepine, etc.) that affect its plasma concentration, thereby indirectly affecting the kinetic properties of levodopa. Therefore, if these drugs need to be used together, the efficacy and adverse reactions should be strictly monitored, and the risks should be assessed by a professional doctor.
Finally, attention should also be paid to avoiding conflicts when scheduling medication. Opicapone should be taken once a day within 1 hour after dinner to reduce interference with food, especially protein. In addition, you should avoid taking it at the same time with iron because iron may combine with levodopa and affect its absorption; it is recommended that the time interval between taking the two is at least 2 hours. Patients should proactively inform their doctors of all the drugs and health care products they are using to ensure the safety and consistency of the overall treatment plan.
