Sotorasib (Lumakras®): The First KRAS Inhibitor Revolutionizing Non-Small Cell Lung Cancer Treatment
I. Overcoming the "Undruggable" Target
For decades, KRAS was considered the holy grail of oncology due to its lack of traditional drug-binding pockets. Affecting approximately 25% of solid tumors, the KRAS G12C mutation is a key driver in non-small cell lung cancer (NSCLC). Amgen’s Sotorasib (Lumakras®) utilizes a novel covalent-binding mechanism to lock the KRAS protein in its inactive state, effectively silencing the growth signals in cancer cells.

II. Clinical Evidence: The CodeBreaK Trials
The pivotal Phase II CodeBreaK 100 study reported an Objective Response Rate (ORR) of 41% and a median Overall Survival (OS) of 12.5 months in pre-treated NSCLC patients. The subsequent Phase III CodeBreaK 200 trial demonstrated superior efficacy over docetaxel chemotherapy, with a median Progression-Free Survival (PFS) of 11.1 months. Notably, Sotorasib has shown promising intracranial activity against brain metastases.
III. Administration and Safety
The recommended dose is 960 mg orally once daily. While generally well-tolerated, common adverse events include diarrhea, nausea, and fatigue. Patients should be monitored for hepatotoxicity and must report any symptoms of interstitial lung disease (ILD), such as shortness of breath, immediately.
