依普利酮(Planep)注意事项
Eplerenone is a new selective aldosterone receptor antagonist used for the treatment of hypertension. What should patients pay attention to when using eplerenone (Planep)?
1. Eplerenone cannot be combined with potassium supplements, potassium-containing salts or contraindicated drugs (potassium-sparing diuretics, strong inhibitors of CYP450 liver enzymes).
2. Non-steroidal anti-inflammatory drugs and lithium can affect the blood concentration of eplerenone and affect the efficacy.
3. Patients with high serum potassium, type 2 diabetes patients with microalbuminuria, patients with high serum creatinine levels, and patients with muscle clearance <50 mL/min should not take eplerenone.
Adverse reactions of eplerenone (1-10%):
Hyperkalemia (2-10%), increased risk of hyperkalemia in the presence of renal dysfunction.
1-3% of adverse reactions include:
Dizziness, fatigue/malaise, abdominal pain, diarrhea, proteinuria, hypercholesterolemia, hypertriglyceridemia, cough.
Adverse reactions in <1% include:
Abnormal vaginal bleeding, gynecomastia, breast pain.

Hypersensitivity: For all patients: Serum potassium >5.5 mEq/L Initial creatinine clearance ≤30 mL/min Concomitant use of strong CYP3A inhibitors Hypertension: Type 2 diabetes mellitus with microalbuminuria Serum creatinine >2.0 mg/dL in men, >1.8 mg/dL in women Creatinine clearance <50 mL/min Concomitant use of potassium supplements or potassium-sparing diuretics.
Suitable for all patients:
Initial serum potassium >5.5 mEq/L;
Creatinine clearance ≤30mL/min;
Concomitant use of strong CYP3A inhibitors.
Used to treat high blood pressure:
Type 2 diabetes with microalbuminuria;
Serum creatinine >2.0 mg/dL in men and >1.8 mg/dL in women;
Creatinine clearance <50 mL/min;
Eplerenone is indicated for concurrent use of potassium supplements or potassium-sparing diuretics.
