Planep用法用量

Planep is a new generation of selective aldosterone antagonist, developed by Pfizer/Pharmacia. It is used for the treatment of hypertension, heart failure and myocardial infarction. What is the dosage of Planep?

1. Congestive heart failure after acute myocardial infarction: The recommended dose is 50 mg/time, once/day. The initial dose should be 25 mg/time, once/day, and within 4 weeks, the dose should be gradually increased to 50 mg/time, once/day if the patient tolerates it.

2. Anti-hypertension: Planep can be used alone or in combination with other anti-hypertensive drugs. The recommended initial dose for use alone is 50mg/time, once/day. Obvious pressure-reducing effect will appear within four weeks of taking the medicine. If the pressure effect is not obvious, it can be increased to 50mg/time, 2 times/day. Higher dosages are not recommended, otherwise there is an increased risk of adverse reactions such as hyperkalemia.

Planep dosage may need to be adjusted based on potassium levels.

Renal Insufficiency: Contraindicated if CrCl <50 mL/min or serum creatinine >2 mg/dL in men or >1.8 mg/dL in women.

Hepatic Impairment: No dose adjustment required.

Dosage considerations: In patients with post-MI CHF taking moderate CYP3A inhibitors, do not exceed 25 mg once daily; in patients with hypertension taking moderate CYP3A inhibitors, do not exceed 25 mg once daily; for inadequate blood pressure response, the dose may be increased to a maximum of 25 mg twice daily.

Hypertension: Initially 50 mg PO qDay; may increase to 50 mg PO q12hr; complete therapeutic response may require up to four weeks; hyperkalemia may occur at doses > 100 mg/day.

Post-MI Heart Failure: Initially 25 mg PO qDay; if tolerated, may be titrated to a maximum of 50 mg once daily over 4 weeks.

Planep dosage may need to be adjusted based on potassium levels.
